What Is Runner's Knee?

Runner's knee — medically called patellofemoral pain syndrome (PFPS) — is a dull, aching pain around or behind the kneecap, caused by irritation of the patellofemoral joint, where the kneecap (patella) glides in a groove on the thigh bone (femur) as the knee bends and straightens. When the kneecap tracks slightly off-centre in that groove, repeated friction and pressure irritate the joint surface.

It's the most common injury in the sport, affecting an estimated 15–25% of runners at some point. The pain is typically diffuse rather than pinpoint — runners often describe it as "behind" or "around" the kneecap rather than at a specific spot — and classically worsens going down stairs or hills, after squatting, and after sitting with the knee bent for a long time (sometimes called "theatre sign," from the discomfort of sitting through a film).

What Causes Runner's Knee?

PFPS is rarely caused by one single factor — it's usually a combination of biomechanics and training load:

  • Hip weakness. Weak hip abductors and external rotators (particularly the gluteus medius) allow the thigh to rotate inward during the stance phase of running, pulling the kneecap off its normal tracking path. This is the most consistently identified cause in the research — more so than any factor at the knee itself.
  • Quad weakness, especially the VMO. The vastus medialis obliquus (VMO), the inner quad muscle just above the knee, helps pull the kneecap medially to balance lateral pulling forces. When it's underdeveloped relative to the rest of the quad, the kneecap tracks slightly outward.
  • Sudden increases in training load. A rapid jump in mileage, hill work, or speed sessions increases the number of knee flexion cycles under load before the joint has adapted — a classic training-load injury, just like IT band syndrome and shin splints.
  • Downhill running and stairs. Both dramatically increase patellofemoral joint compression forces compared to flat running, which is why they're such reliable pain triggers even in mild cases.

The Runner's Knee Recovery Protocol

Recovery combines short-term load reduction with hip and quad strengthening — the strengthening is what prevents recurrence, not just rest.

Load management

  • Reduce running volume and avoid hills, stairs, and downhill terrain until pain settles
  • Cross-train with cycling (raise the saddle slightly to reduce knee flexion) or swimming
  • Avoid deep squats and lunges until strengthening work is pain-free

Hip and quad strengthening (start immediately, even while symptomatic)

  • Clamshells with resistance band: 3 × 20 each side, daily
  • Single-leg glute bridges: 3 × 15 each leg
  • Step-downs: Standing on a low step, slowly lower the opposite heel toward the floor with control, 3 × 10 each leg
  • Wall sits: Build gradually to 3 × 45 seconds, pain-free
  • Terminal knee extensions: Resistance band anchored behind the knee, straightening the leg against resistance, 3 × 15

Returning to Running After Runner's Knee

Progress conservatively and stay on flat terrain initially:

  • Week 1: 3 × 15–20 minute easy runs on flat, even surfaces
  • Week 2: 3 × 25 minutes, still flat, monitor next-day soreness
  • Week 3: Reintroduce gentle inclines if pain-free
  • Week 4+: Resume normal terrain and rebuild mileage at ≤10% per week

Keep hip and quad strengthening in your routine 2–3 times per week long-term — PFPS has one of the highest recurrence rates of any running injury when strength work is dropped after symptoms resolve.
